Health declaration notes

Keep the declaration simple. It records what the member told you. It does not replace a doctor. Say so in a line on the form, and tell members with a listed condition to check with their doctor before they start. Do not diagnose, and do not store more health detail than you need. If a member lists a condition, note it for the trainer and keep the form somewhere only staff can open.

Collect only what you need (DPDP-aware note)

India's Digital Personal Data Protection Act, 2023 covers personal data. The Rules were notified in November 2025 and the main duties for businesses phase in, with most applying from May 2027, per a law firm's summary of the timeline. Commentators describe consent as the main basis for collecting data, tied to the purpose you state. In practice:

  • Ask only for fields you will use. If you never post letters, skip the address.
  • Say why you collect each item, in plain words on the form.
  • Keep forms in a locked cupboard or a password-protected file.
  • Do not share member numbers or photos with others, including in public WhatsApp groups.
  • Let a member correct or ask you to delete their details when they leave, once you no longer need them.

We have not given legal advice here. Check the official text, or ask an adviser, for what your gym must do.

Paper or app

Paper forms are fine to start. The trouble is finding them later. Many owners type each form into an Excel sheet. In Varzish Mate, a 3-step add-member wizard records the member, the dates and the fee, and each member record has Payments and Attendance tabs, so the paper form is just the signed copy. You can also make a payment receipt from the free tool.

Questions and answers

What fields should a gym membership form have?

Name, phone number, plan and dates, amount paid and how, an emergency contact, a health declaration, and the member's signature or acceptance of your rules. Add more only if you use them.

Do I need Aadhaar or an ID copy from every member?

Most gyms do not need it to sell a membership. Collect only what you actually use. If you keep ID copies, you have to protect them.

Is a health declaration compulsory?

Not as a rule we can point to. It is a common practice that records what the member told you and shows that they accepted the risks of exercise.

How long should I keep member forms?

Decide a period, tell members, and delete or return forms when the period ends. Ask your adviser if you are unsure.
